NOTE: Health & Fire Application fees will be charged at time of reservation. JC will be sending your filled out applications to the health and fire depts together along with one master check written by JC Promotions for the fees. ***Make sure to send in your applications as soon as you are approved for a space.

• Health Dept. Food Vendor Permit Application Link-Fee $35- (This permit is required for all food or edible product vendors)
• Fire Department Application Link-Fee $60 Only apply for this permit if you are using propane, charcoal, gasoline or open flame, fryer, gas generator, etc.) Ansul Fire Suppression System -if you use an Ansul Fire Suppression System you will need all required maintenance & inspection certificates to be up to date.
• **You must submit reports with your Fire Application for Kitchen Suppression and Hood cleaning report, Kitchen System Report, Annual Inspection Report for LPG-Gas Containers and Fuel Gas Pipe System.
• All Mobile Trucks must have Alarm install in the truck as per the Fire Code 319.8.5 LP-Gas Alarms

**Health and Fire applications must be downloaded & printed. (links above) You can scan and email or send filled-out permit applications back to JC Promotions as soon as you are approved for an event.

• Certificate of Liability Insurance (COI) is required when serving ANY type of food, including prepackaged items. See the names of all parties that must be included on your COI below.
