  • NOTE: New policy. Health & Fire Application fees will be charged at time of reservation. JC will be sending your filled out applications to the health and fire depts together along with one master check written by JC Promotions for the fees. Make sure to send in your applications as soon as you are approved for a space.
  • Health and Fire applications must be downloaded & printed. (links above) Send filled-out permit applications back to JC Promotions as soon as you have been accepted.
  • Note: If your forms are not received 3-4 weeks before the event your reservation may be canceled and there will be no refunds or credits issued.

Ansul Fire Suppression System if you use an Ansul Fire Suppression System you will need all required maintenance & inspection certificates to be up to date. Certificate of Liability Insurance (COI) is required when serving ANY type of food, including prepackaged items. See the names of all parties that must be included on your COI below.

Certificate of Insurance (COI):
Must be received by JC Promotions via email, fax, or mail…no later than 10 business days before your event.
