Online Food Vendor Application Fannywood Pop Up
Health Dept. Food Vendor Permit Application Link-Fee $75
Fire Department Application Link– Tent set up-Fee $80 – Only apply for this permit if you are using propane, charcoal, gasoline or open flame, fryer, gas generator, etc.)
Fire Department Application Link– Mobile Food Truck-$80 – This is the application for mobile food trucks/trailers.

Ansul Fire Suppression System if you use an Ansul Fire Suppression System you will need all required maintenance & inspection certificates to be up to date.
Certificate of Liability Insurance (COI) is required when serving ANY type of food, including prepackaged items. See the names of all parties that must be included on your COI below.

General Information:
Health and Fire applications must be downloaded & printed. (links above) Applications must be returned to JC Promotions filled out, with all necessary paperwork required and payment by Sept. 18, 2024.
Note: If your forms and payment are not received by Oct. 9th your reservation may be canceled and there will be no refunds or credits issued.

Permit applications/payments: return to JC by any of these methods:

Certificate of Insurance (COI):
Must be received by JC Promotions via email, fax, or mail…no later than 2 weeks prior to the event, by (Sept. 18th)
